From Monday 2nd September 2019 you surgery will be open:

At 8am Monday to Friday

On Thursday Afternoon from 2pm until 6.30pm

On Monday's your surgery will offer extended hours appointments between 6.30pm & 7.30pm. These appointments will be available for patients who cannot attend the surgery in core hours. (between 8am & 6.30am)

New Patient Registration

registrationAnyone who lives within our practice boundary can register as one of our patients. If you are unsure if you live within our boundary please visit

If you wish to register, please see one of our receptionists who will ask you to complete a  GMS1 form and a Patient Health Questionnaire,  You can download and print out these forms using the links below.  We will need to have details of your previous or current GP, and your NHS number, as well as the documentation described below.

Only one adult family member need attend the surgery to register a family provided that the address details are the same for them all.

Proof of Address

This will verify that you are living within our practice area. This evidence should be:

  • A utility bill            
  • Tenancy agreement
  • Bank statement     
  • Mortgage statement
  • Council tax bill        
  • Other official correspondence verifying address

A photocopy will be taken of the proof of address evidence for each patient.

Online Services  

You may also wish to register of our online services.  This includes the ability to book and cancel appointments, order repeat medication and view information from your medical records. 

Photo ID will be required.  An information leaflet and application form can be collected from Reception. 

Change of Address and Telephone Numbers

We ask that you notify us of any change to your address or telephone numbers. We can only use the current details and if they have not been updated then we may not be able to contact you when necessary. 

If you move out of our practice area you may be asked to register at a nearer doctor. You will be given 28 days to do this.
